To enroll:
1. Registration form must be filled out
2. Make an appointment to enroll your child M-F 8:30-5pm
3. Pay Registration fee of $50
4. Pay $5 per week deposit (Includes Financial Assistance or Social Services Participation)
Bring to your appointment:
1. Current Immunization Record
2. Current Physical Exam signed by health care provider
3. Birth Certificate
