Step 1: Click the Leaders Club Application
Step 2: Download the application to your computer/phone.
Step 3: Locate the application on your computer/phone and click each text box to type your answer.
Step 4: Once the application is complete, return the application by email to jakayse@martinsvilleymca.com or you may turn in to the front desk at the Martinsville YMCA.
Make sure you click Leaders Club under Campaign type.
